* mh/folder.c: Improve compatibility with nmh: if a folder is
specified along with -all, set the recursion depth to 2 (unless
-recurse has also been given).
(print_header,print_total): Default to -1
(recurse): Removed in favor of max_depth.
(has_folder,max_depth): New variable.
(OPTION_IS_SET): New macro.
(opt_handler): Updated for new variable usage.
(_scan): Depth argument is now size_t
Take into account max_depth.
(action_print): Use OPTION_IS_SET with the three-state variables
(main): Properly set/clear show_all and max_depth.
